LSSI is offering a part-time position for Part-time Counselor/Case Manager for the RISE Program to provide intensive community based case management services for individuals with serious mental illness and substance use to promote independence in the community.


Duties and Responsibilities:

  • Provide and coordinate a continuum of treatment service
  • Outreach and engage members and address MH/SA services as needed
  • Carry a caseload of clients providing supportive counseling and concrete services assigned by supervisor and operate as part of a treatment team.
  • Ensure all documentation is complete to meet Rule 140 &/or MCO requirement
  • Transportation of clients in personal vehicles, home visits, crisis intervention and assess for safety is required.
  • Certification in IMCANS


Qualifications
:

  • Bachelors or Masters degree
  • Experience with homeless population, preferred
  • Must have excellent written and verbal communication skills
  • Must have a valid Illinois Drivers License, access to a fully insured motor vehicle and proof of insurance
  • Must pass a State of Illinois criminal and DCFS background check
